Ibuprofen, Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.

Here's the honest picture: the concern is that neem leaf extract might block a liver enzyme called CYP1A2, which could slow the breakdown of certain drugs and let them build up. But this worry is based only on lab dish (in vitro) studies, and it hasn't actually been seen in real people. On top of that, neither ibuprofen nor acetaminophen relies much on CYP1A2 to be cleared, so this particular pairing is unlikely to cause you trouble.

If anything ever did build up, you'd watch for things like stomach upset or irritation from the ibuprofen. The practical move is simple: keep doses as directed, and check in with your pharmacist or doctor before adding concentrated neem supplements, since swallowed neem can be risky on its own.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.

Here's the gist with Advil Dual Action, which pairs ibuprofen with acetaminophen. The ibuprofen part is partly cleared by a liver enzyme called CYP2C9. In test tube studies, neem leaf extract seems to slow that same enzyme down. If that held true in people, ibuprofen could hang around a bit longer and its effects, both good and bad, could be amplified. With ibuprofen that might mean more stomach upset, heartburn, a higher chance of irritation or bleeding in the gut, or fluid retention.

The honest part: this is only lab-based research and has not been seen in actual patients. So I wouldn't lose sleep over it. Still, if you plan to use neem supplements regularly alongside this, give your pharmacist or doctor a quick heads-up first.
