Chlorpheniramine, Methscopolamine, Phenylephrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product (a sedating antihistamine plus a drying agent and a decongestant) is partly cleared by a liver enzyme called CYP3A4. Lab studies suggest neem leaf extract can slow that enzyme down. If that happened in real life, more of the medicine could linger in your body, which might mean stronger effects like extra drowsiness, dry mouth, blurry vision, constipation, trouble urinating, or a faster, more pounding heartbeat from the phenylephrine.
Here's the reassuring part: this is only seen in test tubes so far, never reported in actual patients, so the real-world risk looks low. None of these are prodrugs, so the worry is buildup, not loss of effect. Still, if you take concentrated neem supplements regularly, mention it to your pharmacist before combining.
