Some potentially nephrotoxic drugs include cyclosporine (Neoral, Sandimmune); aminoglycosides including amikacin (Amikin), gentamicin (Garamycin, Gentak, others), and tobramycin (Nebcin, others); nonsteroidal anti-inflammatory drugs (NSAIDs) including ibuprofen (Advil, Motrin, Nuprin, others), indomethacin (Indocin), naproxen (Aleve, Anaprox, Naprelan, Naprosyn), piroxicam (Feldene); and numerous others.

Here's the concern with pairing Siberian cocklebur with your Panlor (acetaminophen, caffeine, and dihydrocodeine). The cocklebur fruit contains naturally toxic compounds that can be hard on the kidneys. Acetaminophen, especially at higher doses or over time, also puts some stress on the kidneys (and the liver). Putting these two together could add up and strain kidney function more than either alone.

This is mostly a theoretical, additive worry, but kidney injury is serious, so I'd take it to heart. Watch for signs like swelling, less urination, or unusual tiredness. Honestly, given that cocklebur can cause real poisoning, I'd skip it unless an expert is guiding you. Please run this by your pharmacist or doctor before combining them.

Siberian cocklebur can adversely affect the liver. It has been linked to many cases of hepatotoxicity and some cases of liver failure. Theoretically, concomitant use with other potentially hepatotoxic drugs might increase the risk of developing liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (TriCor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo).

Here's the thing to know about this combination: Siberian cocklebur has been tied to real cases of liver injury, and even liver failure, in people who took it. The product you asked about contains acetaminophen (Tylenol), which your liver also has to process. When you stack two things that can stress the liver at the same time, you raise the odds of damage.

So this isn't about the cocklebur changing how your pain medicine works. It's about doubling up on liver burden. Watch for warning signs like yellowing of the skin or eyes, dark urine, belly pain on the upper right side, or unusual tiredness. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first, especially since this herb carries its own poisoning risk.
