Here's the gist: Xultophy combines insulin degludec (a long-acting insulin) with liraglutide (a GLP-1 medicine). Both already work to bring your blood sugar down. Neem, on its own, may have a mild blood-sugar-lowering effect too. So stacking them together could push your glucose lower than intended. This isn't about neem changing how Xultophy is broken down, it's simply two things working in the same direction at once.

If your sugar drops too low, you might feel shaky, sweaty, dizzy, confused, or very hungry. Keep in mind the evidence here is mostly from lab and animal work, so it's a possible concern, not a sure thing. Still, if you'd like to try neem, check with your pharmacist or doctor first and watch your readings closely.
