Ledipasvir, Sofosbuvir falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist with Harvoni (ledipasvir/sofosbuvir) and neem. Both pieces of Harvoni rely on a little cellular pump called P-glycoprotein to move them around your body. In lab dish studies, neem leaf extract seemed to slow that pump down. If that happened in a person, more of the medicine could hang around, which in theory might mean a bit more of the usual effects like tiredness, headache, or nausea.

The honest part: this is only seen in test-tube and animal research, never reported in actual patients, so it's very much a maybe, not a known problem. Since clearing hepatitis C matters and Harvoni is a fixed course, I'd just loop in your pharmacist or doctor before adding a concentrated neem supplement.

What Is Ledipasvir and Sofosbuvir Used For?

Brand name: Harvoni

Ledipasvir and sofosbuvir treats chronic hepatitis C virus (HCV) infection in adults and pediatric patients 3 years of age and older. It is approved for HCV genotypes 1, 4, 5, and 6 in patients without cirrhosis or with compensated cirrhosis (mild-to-moderate liver scarring that the liver is still managing), and for genotype 1 with decompensated cirrhosis (advanced liver disease) when used together with ribavirin.

It is also approved for liver transplant recipients with HCV genotype 1 or 4 infection who do not have cirrhosis or have compensated cirrhosis, in combination with ribavirin. Both Harvoni and the generic ledipasvir and sofosbuvir tablets share these indications.
