Aspirin, Omeprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist with the aspirin-omeprazole combo (Yosprala) and neem. In lab dishes, neem leaf extract appears to slow down a liver enzyme called CYP3A4, which helps process the omeprazole part of this medicine. If that slowdown happened in people, omeprazole could build up a bit and act more strongly. The aspirin component isn't really cleared by this pathway, so it's the omeprazole side we'd watch.

Honestly, I wouldn't lose sleep over this one. It's only been seen in test-tube studies, never reported in actual patients, so it's largely theoretical. Even if it did occur, omeprazole is quite forgiving. Still, since neem can be risky when swallowed, please run it by your pharmacist or doctor before starting.

Here's the gist with Yosprala (the aspirin and omeprazole combo). Lab tests suggest neem leaf extract can slow down a liver enzyme called CYP2C9, which helps clear part of this medication. If that enzyme runs slower, drug levels could drift a little higher than usual. For the aspirin side, that might mean a touch more stomach irritation or bruising and bleeding tendency, and for omeprazole, slightly stronger acid-lowering effects.

I want to be honest about how soft this is. This comes only from test-tube research and has never been reported in actual people, so it's more theory than proven concern. Concentrated neem supplements are more likely to matter than tiny amounts in toothpaste. If you take both, just give your pharmacist or doctor a heads-up.
