Ulipristal Acetate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the situation with ella (ulipristal). Your body relies on an enzyme called CYP3A4 to break this medicine down. Lab studies suggest neem leaf extract may slow that same enzyme. If that happened in people, ulipristal could linger a bit longer and its levels could rise slightly, possibly nudging up common effects like headache, nausea, belly cramps, or changes to your next period.

The honest truth is this is theoretical. It has only been seen in test-tube research, never reported in real people, so I wouldn't lose sleep over it. Since ella is a one-time emergency dose, any overlap is brief. Still, if you take neem supplements, give your pharmacist a heads-up so we can make sure your backup birth control works as intended.
