Why this applies to Vinorelbine
Vinorelbine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body clears vinorelbine using a liver enzyme called CYP3A4. In lab dish studies, neem leaf extract seems to slow that same enzyme down. If that were to happen in a real person, vinorelbine could build up to higher levels than expected, which may mean stronger chemo side effects like a bigger drop in your white blood cell counts (raising infection risk), worse nausea, fatigue, constipation, or nerve tingling in the hands and feet.
I want to be honest about the evidence, though. This has only been seen in test tubes, never reported in actual patients, so it's theoretical. Still, vinorelbine is potent chemotherapy with little room for error, so please don't add neem without first checking with your oncology team or pharmacist.

What Is Vinorelbine Injection Used For?
Brand name: Navelbine
Vinorelbine Injection is used to treat non-small cell lung cancer (NSCLC) that has spread locally or to other parts of the body (metastatic). It can be used on its own or combined with another chemotherapy drug called cisplatin.
When used with cisplatin, Vinorelbine Injection is a first-line treatment — meaning it can be given as the initial chemotherapy for locally advanced or metastatic NSCLC. As a single agent, it is used for metastatic NSCLC.
