Why this applies to Vorapaxar
Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with neem and vorapaxar (Zontivity). Vorapaxar is cleared from your body mainly by a liver enzyme called CYP3A4. In lab dish studies, neem leaf extract seems to slow that enzyme down. If that happened in real life, your body might clear vorapaxar more slowly, letting its levels creep up. Since vorapaxar is a blood thinner you're already taking alongside aspirin and clopidogrel, the main worry would be extra bleeding or bruising, things like nosebleeds, bleeding gums, or blood in your urine or stool.
The key thing to know: this is only a theoretical concern from test-tube research. It hasn't been reported in actual people. Still, because you're on triple therapy where bleeding risk already matters, I'd run any neem supplement by your pharmacist or doctor first.

What Is Vorapaxar Used For?
Brand name: Zontivity
Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.
Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
