Neem and Omalizumab

Here is the interaction we found between these two, with the evidence behind it.

IMMUNOSUPPRESSANTS

Why this applies to Omalizumab

Theoretically, neem might decrease the effectiveness of immunosuppressants.
Animal research suggests that neem might have immunostimulant effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Xolair (omalizumab) and neem. Xolair is an injectable antibody that calms down part of your immune system's allergic response, which is how it cuts asthma flares, helps with nasal polyps, and tames chronic hives. The theory on file is that neem may gently stimulate the immune system, which could work against what Xolair is trying to do and make it less effective.

I want to be honest about how thin this evidence is. It comes from lab and animal studies, not from people, so it's mostly theoretical. Still, if you rely on Xolair and you notice more wheezing, breathing trouble, or returning hives after starting neem, that's worth a call. Please loop in your doctor or pharmacist before adding neem rather than stopping anything on your own.

About Neem

What is Neem?

Neem
  • Skin conditions
  • Dental and gum health

  • Digestive upset
  • Blood sugar support
  • Insect repellent/pesticide

Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.

About Omalizumab Injection

What Is Omalizumab Injection Used For?

Brand name: Xolair

Xolair is used for several allergy-driven conditions. It treats moderate to severe persistent asthma in adults and children 6 and older whose symptoms aren't controlled by inhaled steroids alone and who test positive for a year-round airborne allergen. It is also used as an add-on maintenance treatment for chronic rhinosinusitis with nasal polyps (ongoing sinus inflammation with polyp growths) in adults 18 and older who haven't responded well to nasal corticosteroids.

Xolair is also approved to reduce allergic reactions — including anaphylaxis — from accidental food exposure in adults and children aged 1 and older with IgE-mediated food allergy. It must be used alongside strict food avoidance, and it is not a substitute for emergency treatment like epinephrine. Finally, Xolair treats chronic spontaneous urticaria (persistent hives with no identified trigger) in adults and adolescents 12 and older who still have symptoms despite antihistamine treatment.
