Here's the gist: your liver turns adrafinil into modafinil, the active stimulant, and modafinil is cleared by an enzyme called CYP3A4. Telaprevir is a strong blocker of that same enzyme. So when the two are taken together, modafinil may not break down as quickly and could build up to higher levels than expected. That can mean stronger or longer-lasting stimulant effects, like trouble sleeping, jitteriness, a racing or pounding heart, anxiety, or headache.

One important note: this interaction is theoretical, based on how the drugs behave on paper rather than real-world reports, so the actual impact is uncertain. Since adrafinil is unregulated and hard on the liver (and you'd be using telaprevir for liver disease), please run this past your doctor or pharmacist first.

Here's the concern with pairing these two. Your body turns adrafinil into modafinil, and modafinil can mildly speed up a liver enzyme called CYP3A4. That same enzyme helps break down telaprevir, your hepatitis C medication. If that breakdown gets nudged faster, you could end up with lower telaprevir levels in your blood.

Why that matters: telaprevir's whole job is to lower the amount of hepatitis C virus in your body, and it needs to stay at a steady, strong level to work. Weaker levels could mean the treatment doesn't suppress the virus as well, which is a real problem during HCV therapy. The good news is this interaction is theoretical, based on how the drugs behave rather than reported cases. Still, please run it by your doctor or pharmacist before combining them.

About Telaprevir

What Is Telaprevir Used For?

  • Telaprevir is used along with two other medications (ribavirin [Copegus, Rebetol] and peginterferon alfa [Pegasys]) to treat chronic hepatitis C (an ongoing viral infection that damages the liver) in people who have not yet been treated for this condition or whose condition could not successfully be treated with ribavirin and peginterferon alfa alone. Telaprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Telaprevir may not prevent the spread of hepatitis C to other people.
