Why this applies to Sofosbuvir
Sofosbuvir falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses a tiny pump called P-glycoprotein to move sofosbuvir (Sovaldi) around and help clear it. In test-tube studies, neem leaf extract seems to slow that pump down. If that happened in people, a bit more sofosbuvir could stay in your system, which in theory might mean more of its usual effects or side effects, like tiredness, headache, or nausea.
I want to be honest about how shaky this is. This is based only on lab research, not real patients, so we truly don't know if it matters in everyday life. Since you're treating hepatitis C and want this medicine working just right, it's worth a quick chat with your pharmacist or doctor before adding concentrated neem supplements.

Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. About Sofosbuvir
What Is Sofosbuvir Used For?
Brand name: Sovaldi
Sovaldi (sofosbuvir) is used to treat chronic hepatitis C virus (HCV) infection. In adults, it treats genotype 1, 2, 3, or 4 HCV — with or without compensated cirrhosis (mild liver scarring that the liver is still managing). It is always used as part of a combination regimen with other antiviral medicines.
In children 3 years and older, Sovaldi is approved for genotype 2 or 3 HCV, with or without compensated cirrhosis, in combination with ribavirin. Sovaldi is also used with ribavirin in adults with HCV-related liver cancer (hepatocellular carcinoma) who are waiting for a liver transplant, to help prevent HCV from returning after transplant.
