Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Tzield (teplizumab) is an infusion given to help delay type 1 diabetes by calming the part of your immune system that attacks insulin-making cells. The whole point of the treatment is to quiet that immune response. Neem works in the opposite direction here. In animal and lab studies, neem appears to stimulate the immune system, so the worry is that it could push against what Tzield is trying to do and make the medicine work less well.

I want to be honest about the strength of this: it is theoretical and based only on animal and test-tube research, not human studies, so it is far from proven. Still, since Tzield is a serious, time-limited therapy, I would not add a neem supplement without first checking with your doctor or pharmacist.

From our pharmacist’s overview on the Teplizumab-mzwv page

Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.

About Teplizumab-mzwv injection

What Is Teplizumab-mzwv injection Used For?

Brand name: Tzield

Tzield is used to delay the onset of Stage 3 type 1 diabetes (T1D) in adults and children aged 1 year and older who already have Stage 2 T1D — meaning their immune system is attacking insulin-producing cells, but they don't yet have overt high blood sugar symptoms.

Tzield is also used to slow the loss of the body's own insulin production in children and teenagers aged 8 to 17 years who have recently been diagnosed with Stage 3 T1D. This second use is approved under accelerated approval, meaning continued approval depends on results from ongoing confirmatory studies.
