Troglitazone (Rezulin) is a diabetes medicine that helps your body use insulin better and brings blood sugar down. The thing to know here is that neem can also nudge blood sugar lower on its own. Stack the two together and the effects may add up, so your sugar could dip further than you expect. That can show up as shakiness, sweating, dizziness, hunger, or feeling lightheaded.

Honestly, this is mostly based on lab and animal work plus a few small human studies, so it is more of a watch-and-be-aware situation than a firm warning. Food amounts in toothpaste or skin products are unlikely to matter. If you take concentrated neem supplements, check in with your pharmacist or doctor first and keep an eye on your sugar readings.
