Neem and Semaglutide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIDIABETES DRUGS

Why this applies to Semaglutide

Semaglutide falls under ANTIDIABETES DRUGS — the group Neem interacts with.

Neem might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Some clinical research shows that neem can lower blood glucose levels in adults with type 2 diabetes, including those already taking metformin.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: both neem and your semaglutide (Ozempic, Wegovy, or Rybelsus) can push your blood sugar down. Semaglutide already lowers blood sugar by helping your pancreas release insulin at the right times. If neem adds its own glucose-lowering nudge on top of that, the two together could drop your numbers lower than you'd expect. That can show up as shakiness, sweating, dizziness, hunger, or feeling foggy or weak.

I'll be straight with you, though: the evidence here is on the weaker side, mostly lab and small studies, so this is more a 'keep an eye out' than a panic situation. If you do take neem, check your blood sugar a little more often at first and let your pharmacist or doctor know before you start. Don't change your semaglutide dose on your own.

From our pharmacist’s overview on the Semaglutide page

“Semaglutide (Ozempic, Rybelsus, Wegovy) lowers blood sugar, and so do most of the 256 supplements on this page, which is why I read this list with real vigilance rather than a shrug. Nearly all of the entries either add to its glucose-lowering effect and raise the risk of hypoglycemia, or push sugar the other way and blunt it, as niaci…”

About Neem

What is Neem?

Neem

  • Blood sugar support
  • Insect repellent/pesticide

Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.

About Semaglutide

What Is Semaglutide Used For?

Brand names: Ozempic, Rybelsus, Wegovy

Semaglutide is available as Wegovy (injection and tablet) and Ozempic (injection), and each product has its own approved uses. Wegovy injection is approved — alongside diet and exercise — to reduce excess body weight in adults and in children aged 12 and older with obesity, to lower the risk of serious heart events (heart attack, stroke, or cardiovascular death) in adults with obesity or overweight who also have established heart disease, and for treatment of a liver condition called MASH (metabolic dysfunction-associated steatohepatitis, formerly called NASH) with moderate-to-advanced scarring in adults. Wegovy tablets share the cardiovascular risk reduction and weight management uses in adults, but are not approved for MASH or for use in children. Ozempic injection is approved to improve blood sugar control in adults with type 2 diabetes (alongside diet and exercise) and to reduce the risk of major cardiovascular events in adults with type 2 diabetes who also have established heart disease.

All forms are used alongside a reduced-calorie diet and increased physical activity. Ozempic is not approved for type 1 diabetes. The MASH approval for Wegovy injection is an accelerated approval, meaning continued approval may depend on results from a confirmatory study.
