Grepafloxacin falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.

Here's the quick version: your body uses a liver enzyme called CYP1A2 to help clear grepafloxacin (Raxar), an antibiotic. Lab studies suggest neem leaf extract may slow that same enzyme down. If that happened in a person, grepafloxacin could build up a bit higher than expected, which in theory might mean more of its usual side effects, like nausea, dizziness, or a longer QT interval (a heart rhythm change this particular drug is known for).

Now, the honest part: this is based only on test-tube research, and it has never actually been reported in real people. So I wouldn't lose sleep over it, but since this drug affects heart rhythm, it's worth a quick check with your pharmacist or doctor before combining them.
