CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Hydrocortisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with moringa and hydrocortisone. Your liver uses an enzyme called CYP3A4 to help break hydrocortisone down, and moringa might nudge that enzyme in either direction. If it slows the enzyme, hydrocortisone could build up a bit (think more fluid retention, higher blood sugar, mood changes, or trouble sleeping). If it speeds the enzyme up, the steroid could clear faster and work less well, which matters a lot if you take it to replace hormones your body cannot make.
The catch is that this is mostly lab and animal data, and one human study showed no real change. So food amounts of moringa likely aren't a concern. Before adding a concentrated moringa supplement, check in with your pharmacist or doctor.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the basic idea with moringa and your hydrocortisone. Your body uses a kind of cellular pump called P-glycoprotein to help move certain drugs around and clear them out. In lab dish studies, concentrated moringa leaf extract seemed to slow that pump down. If that happened in a real person, a bit more hydrocortisone could linger in the body, which in theory could nudge up the usual steroid side effects like fluid retention, higher blood sugar, mood changes, or trouble sleeping.
I want to be honest with you, though: this is theoretical. It has only been seen in the lab, never reported in actual patients, so the risk looks low. A sprinkle of moringa in food is very unlikely to matter; concentrated supplement doses are the bigger question. Before starting a moringa supplement, just run it by your pharmacist or doctor.

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).
It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
