CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Prednisolone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: your body uses an enzyme called CYP3A4 to help break down prednisolone, and moringa might nudge that enzyme one way or the other. The tricky part is that lab studies disagree. Some hint that moringa slows this enzyme (which could let prednisolone build up, meaning more side effects like mood swings, trouble sleeping, higher blood sugar, or stomach upset), while others suggest it speeds the enzyme up (which could make prednisolone work less well).
The good news is this is mostly test-tube science, not real patients. A human study with a similar medication showed no meaningful change. Food amounts of moringa are unlikely to matter much. Still, since the direction is uncertain, check with your pharmacist or doctor before adding concentrated moringa supplements.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the gist with moringa and prednisolone. Your body uses a tiny pump called P-glycoprotein to help move certain medications out of cells and into urine. Lab studies (in test tubes and animals, not in people) suggest moringa leaf extract may slow that pump down. In theory, that could let a bit more prednisolone hang around, which might mean slightly stronger steroid effects or more of the usual side effects like trouble sleeping, mood changes, a bigger appetite, fluid retention, or higher blood sugar.
I want to be honest though: this is theoretical and hasn't actually been seen in humans. Food amounts of moringa in cooking are very unlikely to cause trouble; concentrated supplements are the bigger question mark. If you're taking prednisolone regularly, just run the moringa supplement past your pharmacist or doctor first.
