CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Apixaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with moringa and Eliquis (apixaban). Your body partly clears apixaban using a liver enzyme called CYP3A4. In lab studies, moringa has pulled in two different directions: some research suggests it slows this enzyme down, while other research suggests it speeds it up. Apixaban is an active drug (not a prodrug), so if moringa slowed clearance, more could build up and raise bleeding risk (easy bruising, nosebleeds, blood in urine or stool). If it sped clearance up, the drug could weaken and protect you less from clots.
The key point is that this is theoretical, based on test-tube data, and a small human study showed no real change. Since apixaban is a blood thinner where balance matters, just let your pharmacist or doctor know before adding moringa supplements.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Eliquis (apixaban). Your body partly relies on a transporter called P-glycoprotein to move this blood thinner out of your system. In lab and animal studies, moringa leaf extract appears to slow that transporter down. In theory, that could let apixaban build up a bit more than usual, which would mean a stronger blood-thinning effect and a higher chance of bruising or bleeding, such as nosebleeds, bleeding gums, or blood in the urine or stool.
I want to be straight with you: this is theoretical, based only on test-tube research, and it hasn't actually been seen in people. A sprinkle of moringa in food is unlikely to matter, but concentrated supplement doses are less predictable. Since apixaban leaves little room for error, please run it by your pharmacist or doctor before combining them.

About Apixaban
What Is Apixaban Used For?
Brand name: Eliquis
Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.
Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
