CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Doxorubicin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture: doxorubicin is partly handled by a liver enzyme called CYP3A4, and moringa might nudge that enzyme one way or the other. The catch is that lab studies disagree. Some suggest moringa slows CYP3A4 (which could let doxorubicin build up and raise the risk of its usual side effects, like nausea, mouth sores, low blood counts, or heart strain), while other studies suggest it speeds the enzyme up (which could weaken the chemo). We simply can't say which wins.
Because this is only test-tube and animal data, and a human study with another drug showed no real change, I wouldn't panic. But chemotherapy is serious business, so please run any moringa supplement past your oncologist or pharmacist before using it.

In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the basic idea with moringa and doxorubicin. Your body uses a kind of cellular pump called P-glycoprotein to move certain drugs around and help clear them out. Doxorubicin is one of the medicines that pump handles. In lab (test tube) studies, moringa leaf extract seemed to slow that pump down. In theory, that could let a bit more doxorubicin linger in the body, which might mean stronger effects and a higher chance of side effects like low blood counts, mouth sores, nausea, or extra strain on the heart.
I want to be honest, though: this has only been seen in lab dishes, not in real people, so we don't know if it matters in practice. Since doxorubicin is serious chemotherapy, please run any moringa use by your oncology team first.
