Caffeine (OTC drug) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest scoop on pairing moringa with a caffeine pill like Vivarin. The concern on file is that moringa might tweak a liver enzyme called CYP3A4, which plays a small part in clearing some drugs. The catch is that the lab studies disagree with each other: some hint moringa slows that enzyme down, others suggest it speeds it up. So we can't even say which direction this would go.
On top of that, caffeine is actually broken down mostly by a different enzyme (CYP1A2), not CYP3A4, so this particular interaction likely matters very little for caffeine. If moringa did somehow raise caffeine levels, you might notice more jitteriness, a racing heart, or trouble sleeping. This is theoretical and based only on test-tube work, so don't panic. Food amounts of moringa are especially unlikely to matter. If you regularly use concentrated supplements of both, just run it by your pharmacist.

Your body clears caffeine mainly through a liver enzyme called CYP1A2. Lab studies suggest moringa may rev that enzyme up a bit, which in theory could break caffeine down faster. If that happened, you might notice your usual caffeine pick-me-up wearing off sooner or feeling a little weaker, meaning less of that alertness boost.
Honestly, I wouldn't lose sleep over this one. The concern comes only from test-tube research, not real people, and it's rated a minor interaction. Food amounts of moringa are very unlikely to matter, and even supplement doses may do little. If you rely on caffeine to stay alert and notice a change after adding moringa, just mention it to your pharmacist or doctor.
