Moringa and Testosterone Undecanoate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Testosterone Undecanoate

Testosterone Undecanoate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture: oral testosterone undecanoate is partly handled by a liver enzyme called CYP3A4, and moringa might nudge that enzyme one way or the other. The catch is that the lab studies disagree. Some hint that moringa slows CYP3A4 (which could let testosterone build up a bit), while others suggest it speeds the enzyme up (which could lower your levels). This isn't a true prodrug situation, so the direction simply isn't settled.

The evidence here is weak (mostly test-tube and animal data), and a human study with another CYP3A4 drug showed no real change. Everyday food amounts of moringa are unlikely to matter much. Still, since testosterone dosing is carefully tuned, mention any moringa supplement to your pharmacist or doctor before starting it.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Moringa

What is Moringa?

  • Nutritional supplement
  • Blood sugar support
  • Cholesterol support
  • Inflammation
  • Skin conditions

Moringa is a nutrient-rich plant whose leaves are widely used as a food and supplement, especially in parts of the world where malnutrition is common. Early research hints at possible benefits for blood sugar, cholesterol, and inflammation, but the human evidence is limited and not strong enough to recommend it as a treatment. Talk with your pharmacist or doctor before using moringa, especially if you are pregnant, breastfeeding, or taking medications.

About Testosterone Injection

What Is Testosterone Injection Used For?

Brand names: Andriol, Aveed, Jatenzo, Kyzatrex

Testosterone injections replace testosterone when the body makes too little or none. The label describes this in men with primary hypogonadism (a problem in the testicles) and hypogonadotropic hypogonadism (a problem with the pituitary or hypothalamus signals).

Testosterone Enanthate Injection is also used to stimulate puberty in carefully selected boys with clearly delayed puberty. It may be used secondarily in some postmenopausal women with advanced, inoperable breast cancer that has spread to the bone, under an oncologist’s direction. Aveed is for hypogonadism in men and is not for use in women.
