Why Are There 2 Interactions?
Moringa interacts with whole categories of medication. Acetaminophen, Dextromethorphan, Guaifenesin, Phenylpropanolamine falls into 2 of those categories, so it can interact with Moringa in 2 different ways. CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Dextromethorphan, Guaifenesin, Phenylpropanolamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination cold-and-cough product (Anatuss) contains four ingredients, and one of them, dextromethorphan, is partly broken down by a liver enzyme called CYP3A4. The concern with moringa is purely theoretical right now. Lab studies disagree with each other: some suggest moringa slows CYP3A4 down, others suggest it speeds it up. A small human study with a different CYP3A4 drug showed no real change at all.
So in plain terms, we don't have good evidence that moringa meaningfully changes how this medicine works. If it did slow the enzyme, you might feel a bit more drowsy or dizzy from the dextromethorphan. Food amounts of moringa are very unlikely to matter. If you take concentrated moringa supplements regularly, just mention it to your pharmacist.

This combo cold-and-cough product (Anatuss) pairs a fever and pain reliever, a cough suppressant, an expectorant, and a decongestant. The piece that touches moringa is mostly dextromethorphan, which the body processes partly through a liver enzyme called CYP1A2. Lab and animal studies suggest moringa might speed that enzyme up a little, which could in theory clear the drug faster and make the cough-calming effect a bit weaker.
Honestly, I wouldn't lose sleep over this one. The evidence is only test-tube level and rated minor, and food amounts of moringa are very unlikely to matter. If you take concentrated moringa supplements daily and notice your cold medicine just isn't working as well, mention it to your pharmacist or doctor.
