CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Lidocaine
Lidocaine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: lidocaine is partly broken down by a liver enzyme called CYP3A4, and moringa might nudge that enzyme one way or the other. The catch is the lab studies don't agree. Some hint moringa slows the enzyme down (which could let lidocaine build up a bit), while others suggest it speeds the enzyme up (which could clear lidocaine faster). Lidocaine is not a prodrug, so it doesn't need this enzyme to start working.
Honestly, this is all theoretical and based on test-tube data, not real people. For a patch or cream on your skin, very little lidocaine reaches your bloodstream, so I wouldn't lose sleep over it. Still, it's worth a quick word with your pharmacist if you take concentrated moringa supplements.

What Is Lidocaine Transdermal Patch Used For?

Lidocaine skin patches are applied directly to the painful area to treat post-herpetic neuralgia (PHN) — the persistent burning or aching nerve pain that can remain after a shingles outbreak.
Lidoderm (lidocaine patch 5%) and ZTlido (lidocaine topical system 1.8%) are both indicated for this use in adults. Note that despite their different strengths, one ZTlido topical system provides equivalent lidocaine exposure to one Lidoderm patch.
