CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Glecaprevir, Pibrentasvir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Mavyret (glecaprevir/pibrentasvir) and moringa. Mavyret is partly handled by a liver enzyme called CYP3A4, and moringa has shown mixed signals in test-tube studies. Some lab work hints it might block this enzyme (which could nudge Mavyret levels up, possibly meaning more side effects like headache, tiredness, or nausea), while other lab work suggests it speeds the enzyme up (which could lower levels and risk a weaker response against the hepatitis C virus). This is not a prodrug, so a real drop in levels would mean less benefit, not more.
The key thing: this is theoretical, based only on lab and animal data, and one human study with a similar drug showed no real change. Food amounts of moringa likely don't matter, but concentrated supplements are less predictable. Since finishing your full Mavyret course matters for a cure, please run moringa by your pharmacist or doctor first.

In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the short version on combining moringa with Mavyret (glecaprevir and pibrentasvir). Both of these antiviral ingredients are carried around your body by a transporter called P-glycoprotein. Lab studies (in test tubes, not in people) suggest that moringa leaf extract can slow that transporter down. In theory, that could let more of the drug build up, which might mean a bit more in the way of side effects like tiredness, headache, or nausea.
I want to be clear that this is theoretical. It has never been reported in an actual person, so I wouldn't lose sleep over it. The amount of moringa in cooking is unlikely to matter, while concentrated supplements are the bigger question mark. Before adding a moringa supplement, just run it by your pharmacist or doctor since clearing the virus matters.

What Is Glecaprevir and Pibrentasvir Used For?
Brand name: Mavyret
Mavyret treats acute or chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 in adults and children 3 years and older who have no cirrhosis or have compensated cirrhosis (mild liver scarring that is still functioning).
Mavyret is also approved for adults and children 3 years and older with genotype 1 HCV who were previously treated with either an NS5A inhibitor or an NS3/4A protease inhibitor (two earlier classes of hepatitis C drugs) — but not both at the same time.
