Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill pairs aspirin and caffeine with codeine, and codeine is the piece worth thinking about here. Codeine is a prodrug, meaning your body has to convert it into morphine to give pain relief. That activation happens mainly through an enzyme called CYP2D6, not CYP3A4, so the on-file concern about CYP3A4 only touches a side route that helps clear codeine.
The honest takeaway is that this interaction is theoretical and based on lab studies, and they even disagree on whether moringa speeds up or slows down CYP3A4. One human study showed no real change. So everyday food amounts of moringa are unlikely to matter much. Still, since codeine can cause drowsiness, constipation, and slowed breathing, check with your pharmacist or doctor before pairing it with concentrated moringa supplements.

The piece of this combo pill that interacts with moringa is the caffeine. Your liver uses an enzyme called CYP1A2 to clear caffeine, and lab studies suggest moringa might speed that enzyme up. If that happened in real life, your body could break down caffeine a little faster, so you'd notice less of its usual lift (a bit less alertness, maybe a touch more grogginess). The aspirin and codeine parts aren't really affected by this pathway.
Here's the reassuring part: this is only a theoretical concern based on test-tube and animal research, rated minor. Everyday culinary amounts of moringa are very unlikely to matter. If you take concentrated moringa supplements daily, just mention it to your pharmacist or doctor so they can keep an eye on things.
