Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the wrinkle with this combination pill: the codeine part is a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. The beta-carboline compounds in ayahuasca (harmine and harmaline) may block that same enzyme. If they do, you'd convert less codeine into morphine, so you might get weaker pain relief rather than more.

Now the honest part: this is only seen in lab (test tube) studies, not in real people, so it's theoretical. Ayahuasca itself is also a much bigger safety concern than this enzyme effect. Either way, please don't change anything on your own. Talk it through with your pharmacist or doctor first.

Here's something worth knowing about this combination pill. Of the three ingredients, codeine is the one to watch. Codeine is a prodrug, meaning your body has to convert it into morphine before it does much of anything for pain. Most of that activation happens through CYP2D6, but CYP3A4 also plays a role in handling codeine. The beta-carboline alkaloids in ayahuasca may gently block CYP3A4, which could nudge codeine levels up and add to side effects like drowsiness, constipation, nausea, or slowed breathing.

Honestly, this is theoretical right now, based only on lab studies, not real people. The far bigger concern is that ayahuasca acts as an MAO inhibitor, which can react badly with opioids and caffeine. Please check with your doctor or pharmacist before mixing these.
