Ayahuasca and Ciclesonide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Ciclesonide

Ciclesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your inhaler. Ayahuasca contains plant compounds (beta-carboline alkaloids) that, in lab dish studies, slowed down a liver enzyme called CYP3A4. Ciclesonide is partly handled by that same enzyme, so in theory the brew could let more of the active drug linger in your body than usual.

I want to be honest with you though: this is only a theoretical concern based on test-tube research, and it has not actually been seen in people. Inhaled ciclesonide also reaches the blood in only small amounts, so a big problem is unlikely. Still, more steroid exposure could mean things like oral thrush, hoarseness, or mild steroid side effects. Honestly, the bigger safety issue is ayahuasca's MAOI activity itself. Please talk with your doctor or pharmacist before mixing these.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Ciclesonide page

“Ciclesonide (Alvesco, Omnaris, Zetonna) is a corticosteroid for asthma and nasal allergies, and even though it is inhaled rather than swallowed, its supplement list warrants genuine care. The body clears it through the CYP3A4 enzyme, and 121 of the 166 ingredients on file nudge that pathway one way or the other.”
