Ayahuasca and Chlorpheniramine Maleate, Hydrocodone Bitartrate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Ayahuasca interacts with whole categories of medication. Chlorpheniramine Maleate, Hydrocodone Bitartrate falls into 3 of those categories, so it can interact with Ayahuasca in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

SEROTONERGIC DRUGS

1 of 3

Why this applies to Chlorpheniramine Maleate, Hydrocodone Bitartrate

Chlorpheniramine Maleate, Hydrocodone Bitartrate falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the concern with Vituz (chlorpheniramine and hydrocodone) and ayahuasca. The vine in ayahuasca acts like an MAO inhibitor and, along with its DMT, pushes serotonin levels up. Hydrocodone is an opioid that also nudges serotonin activity, so stacking the two could, in theory, lead to too much serotonin. That's called serotonin syndrome, and it can show up as shivering, sweating, a racing heart, tremor, confusion, nausea, or agitation.

This is based on how the drugs work and a few case reports, not solid studies, so treat it as a possible risk rather than a sure thing. Even so, opioids plus a strong psychoactive brew is a combination I'd never wave through. Please run it by your doctor or pharmacist first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

2 of 3

Chlorpheniramine Maleate, Hydrocodone Bitartrate also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your Vituz combo contains hydrocodone, an opioid, plus chlorpheniramine, an antihistamine. Hydrocodone is partly turned into its stronger pain-relieving form by a liver enzyme called CYP2D6. The beta-carboline alkaloids in ayahuasca (harmine and harmaline) may block that enzyme, at least in lab studies. In theory, that could mean less of hydrocodone's active form, so you might feel less pain relief. The parent drug can still build up, though, and both ingredients already cause drowsiness, so sedation is a real concern.

Honestly, the bigger worry here is that ayahuasca acts as an MAOI, which can react badly with many medicines. This enzyme effect has only been seen in the lab, not in people, but please check with your doctor or pharmacist before mixing these.

Chlorpheniramine Maleate, Hydrocodone Bitartrate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Vituz combines hydrocodone (an opioid cough and pain reliever) with chlorpheniramine (a sedating antihistamine). The hydrocodone part is partly handled by a liver enzyme called CYP3A4. Lab studies hint that compounds in ayahuasca may slow that enzyme down, so in theory the medicine could clear a bit slower and build up. That might mean more drowsiness, dizziness, slowed breathing, constipation, or feeling very foggy.

I want to be honest though: this is only seen in test-tube studies, not in real people, so the risk is uncertain. The bigger worry is that ayahuasca acts as a strong MAOI, and mixing MAOIs with opioids can be genuinely dangerous. Please don't combine these without talking to your doctor or pharmacist first.
