Why this applies to Dextromethorphan

Dextromethorphan falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the thing with this combo: dextromethorphan (the active ingredient in many cough syrups and cold strips) does more than quiet a cough. It also nudges up serotonin in your brain. Ayahuasca works on serotonin too, partly because its vine compounds (harmine and harmaline) act like MAO inhibitors. Stack those two together and you can get too much serotonin activity at once.

That can lead to a reaction called serotonin syndrome, with symptoms like shaking, sweating, a racing heart, confusion, agitation, nausea, or high fever. To be fair, this is rated theoretical and based mostly on how these substances behave, not large studies. But MAOIs plus dextromethorphan is a classic risky pairing, so please don't combine them without talking to your pharmacist or doctor first.

Dextromethorphan also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the thing with cough syrup that contains dextromethorphan: your liver relies on an enzyme called CYP2D6 to break it down and clear it out. The beta-carboline alkaloids in ayahuasca (harmine and harmaline) appear to slow that same enzyme down, at least in lab studies. So in theory, less dextromethorphan gets cleared and more of it can build up, which could mean stronger drowsiness, dizziness, confusion, or feeling spacey.

There's a bigger worry, though. Ayahuasca also acts like an MAOI, and combining MAOIs with dextromethorphan can risk serotonin syndrome (agitation, racing heart, high temperature, muscle twitching). This particular CYP2D6 interaction is only theoretical and hasn't been confirmed in people, but please don't mix these without first checking with your doctor or pharmacist.

Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the bigger picture with these two: ayahuasca's strongest concern with dextromethorphan isn't really about the liver enzyme in our database. The vine in ayahuasca acts like a powerful MAO inhibitor, and dextromethorphan raises serotonin in the brain. Combining the two can push serotonin too high, leading to serotonin syndrome (agitation, fast heartbeat, sweating, shaking, high fever, and confusion). That can be dangerous.

On top of that, the lab-only finding here suggests ayahuasca's beta-carbolines may slow CYP3A4, one pathway that helps clear dextromethorphan. In theory that could let more drug build up, meaning more drowsiness or dizziness. This part is theoretical and only seen in test tubes, not people. Either way, please don't pair these without first checking with your doctor or pharmacist.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
