SEROTONERGIC DRUGS

Carbetapentane, Chlorpheniramine, Phenylephrine falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the thing to know about this combo. Your Ricotuss product has three pieces, and two of them matter here. Carbetapentane (a cough suppressant) has some serotonin-like activity, and phenylephrine is a decongestant that bumps up adrenaline-type signals and can tighten blood vessels. Ayahuasca acts like an MAO inhibitor and raises serotonin, so layering it on top could push things too far.
The worry is a spike in blood pressure, a pounding or racing heart, headache, sweating, and shivering, or in a bad case the cluster of symptoms called serotonin syndrome. This is a theoretical concern based on how these drugs behave, not a long list of real cases, but the potential risk is serious enough that I'd run it by your doctor or pharmacist before mixing them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the short version on combining ayahuasca with this cough and cold combo (Ricotuss). The beta-carboline compounds in ayahuasca can, at least in lab studies, slow down the CYP3A4 enzyme that helps your body clear several medicines. If that happens, the cough suppressant carbetapentane and the antihistamine chlorpheniramine could build up a bit, which might mean more drowsiness, dry mouth, dizziness, or a foggy head than usual.
I want to be honest about the strength here: this is theoretical, based on test-tube research and not yet seen in real people. The bigger concern with ayahuasca is actually its strong MAOI activity mixing with cold medicines, which can affect blood pressure. Please check with your pharmacist or doctor before mixing these.
