Dofetilide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the concern with mixing ayahuasca and dofetilide. The beta-carboline compounds in ayahuasca (like harmine) have been shown in lab studies to block CYP3A4, one of the enzymes that helps clear dofetilide from your body. In theory, slowing that breakdown could let dofetilide levels climb higher than intended.

That matters because dofetilide is a narrow therapeutic index drug, meaning even small increases can be risky. Higher levels raise the chance of a dangerous heart rhythm problem called QT prolongation. To be fair, this enzyme effect has only been seen in test tubes, not in actual people, so it stays theoretical for now. Still, given dofetilide's heart risks (and ayahuasca's own safety issues), please run this by your doctor or pharmacist before combining them.

About Dofetilide

What Is Dofetilide Used For?

Brand name: Tikosyn

Dofetilide capsules are used in two main ways for people with atrial fibrillation (AF) or atrial flutter (AFl) — abnormal, often rapid heart rhythms. First, they can help convert these abnormal rhythms back to a normal heart rhythm. Second, once a normal rhythm has been restored, dofetilide can help maintain it and delay the time before the abnormal rhythm comes back.

Because dofetilide can cause dangerous heart rhythm problems of its own, it is reserved for patients whose atrial fibrillation or atrial flutter causes significant symptoms. It has not been shown to be effective for paroxysmal atrial fibrillation (short, self-terminating episodes). The brand Tikosyn carries the same two approved uses.
