Why this applies to Acetaminophen, Codeine
Acetaminophen, Codeine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the wrinkle with this combo: codeine is a prodrug. Your body has to convert it using an enzyme called CYP2D6 before it actually relieves pain. The beta-carboline compounds in ayahuasca (harmine and harmaline) may block that same enzyme. If CYP2D6 is slowed down, less codeine turns into its active form, which usually means weaker pain relief, not more.
I'll be honest about the evidence here: this is only based on lab studies, and it hasn't actually been shown to happen in people, so it's largely theoretical. That said, ayahuasca carries other serious risks of its own. Please don't start, stop, or change anything on your own. Loop in your doctor or pharmacist first.
