Acetaminophen, Codeine falls under GLUCURONIDATED DRUGS — the group Lemongrass interacts with.

Theoretically, lemongrass might increase the clearance and decrease the levels of glucuronidated drugs.
Animal research shows that lemongrass and its constituent citral induce uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation.

Here's the gist with this combo. Both the acetaminophen (Tylenol) and the codeine in this pill rely partly on a liver process called glucuronidation to get cleared from your body. In lab and animal studies, lemongrass (and its lemony compound citral) seemed to speed up that process. In theory, that could clear these medicines a little faster and lower their levels, which might make the pain relief feel weaker than expected.

I want to be honest though: this is theoretical and based only on test-tube and animal data, not real people. The pinch of lemongrass in food or a cup of tea is very unlikely to matter. Concentrated supplements or the essential oil are where I'd be more cautious. If you take those regularly, just run it by your pharmacist or doctor first.
