Acetaminophen, Codeine falls under CNS DEPRESSANTS — the group Delta-9-tetrahydrocannabinol (thc) interacts with.
Cannabis containing THC can have CNS depressant effects. Combining THC with other CNS depressants might result in additive or synergistic effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist of it. The codeine in Tylenol with Codeine is an opioid, and opioids tend to slow down your brain and body. THC from cannabis can do something similar, leaving you sleepy, foggy, and a little off balance. So when you put the two together, those calming, drowsy effects can stack up on each other.
What might that look like for you? Think extra drowsiness, dizziness, slowed thinking, and trouble concentrating. Driving or operating anything risky becomes a real concern. In larger amounts, opioids can also slow your breathing, and that's the part to take seriously. To be fair, this concern is based on how these substances work rather than firm studies, but it's a sensible caution. Before mixing them, please check with your pharmacist or doctor.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

In vitro research shows that cannabis containing THC can induce the activity of CYP2E1, which might increase the metabolism of CYP2E1 substrates. It is unclear if this effect is due to THC, other constituents, or the combination.

Here's the honest picture: this combo (acetaminophen with codeine) involves two different drugs, and the interaction on file points to the acetaminophen part. THC might speed up an enzyme called CYP2E1, and CYP2E1 is one of the pathways that handles acetaminophen. The worry isn't really about losing pain relief, it's that this pathway can turn a small slice of acetaminophen into a byproduct that's hard on the liver, especially with higher doses or regular alcohol use.
That said, this is theoretical, based only on lab studies, so don't lose sleep over it. Keep acetaminophen within recommended limits and watch the sedation, since both THC and codeine can make you drowsy and foggy. Loop in your pharmacist or doctor before mixing these.
