Why this applies to Amitriptyline, Perphenazine

Amitriptyline, Perphenazine falls under SEROTONERGIC DRUGS — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase levels of serotonin, which may have additive effects with serotonergic drugs.
In vitro and animal research shows that harmaline and harmine, constituents of ayahuasca, strongly and reversibly inhibit monoamine oxidase A and weakly inhibit monoamine oxidase B. The dimethyltryptamine and tetrahydroharmine constituents of ayahuasca have also demonstrated serotonergic activity. A case of serotonin syndrome characterized by tremors, sweating, shivering, confusion, nausea, vomiting, and disorientation has been reported in a 36-year-old male who consumed 100 mL of ayahuasca while taking fluoxetine 20 mg daily. Combining serotonergic drugs with ayahuasca might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders.

Here's the concern with this combo. The amitriptyline in Triavil raises serotonin levels in your brain, and ayahuasca does the same thing through its MAOI-acting compounds (harmine and harmaline) plus its DMT. Stack those two together and you can push serotonin too high, which risks a reaction called serotonin syndrome. Watch for shivering, sweating, a racing heart, tremors, agitation, confusion, or nausea.

This warning is theoretical, based on how these compounds behave rather than lots of patient cases, but it lines up with a real report involving an antidepressant and ayahuasca. Mixing a potent MAOI source with a tricyclic is genuinely risky, so please don't combine them without first talking to your doctor or pharmacist.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

This combination pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic), and both rely partly on the liver enzyme CYP2D6 to be cleared from your body. Lab studies suggest the beta-carboline compounds in ayahuasca (harmine and harmaline) can block that enzyme. In theory, that could let these drugs build up, leading to stronger effects like heavy drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, a faster heartbeat, or muscle stiffness.

I want to be honest: this is based only on test-tube work and has not been shown in real people, so it stays theoretical. But there's a bigger concern. Ayahuasca acts as an MAO inhibitor, and mixing MAOIs with amitriptyline can be genuinely dangerous (serotonin syndrome). Please talk with your doctor or pharmacist before combining these.

Here's the thing to know about combining ayahuasca with Triavil (which pairs amitriptyline, an antidepressant, with perphenazine, an antipsychotic). Lab studies hint that the beta-carbolines in ayahuasca may slow down CYP3A4, one of the liver enzymes that helps clear these medicines. If that happened in a person, more drug could linger in your body, nudging up side effects like heavy drowsiness, dry mouth, constipation, blurry vision, dizziness on standing, or an irregular heartbeat.

But I want to be honest about the strength of this: it's only seen in test-tube research and has never been confirmed in real people, so this part is theoretical. The bigger worry is separate. Ayahuasca acts as an MAOI, and mixing MAOIs with amitriptyline can cause a dangerous serotonin reaction. Please talk with your doctor or pharmacist before going near this combination.
