Ciclesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Devil's Claw interacts with.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your inhaler. Ciclesonide is actually a 'pro-drug,' meaning your lungs activate it into its working form. After that, your body uses an enzyme called CYP3A4 to help clear it out. Devil's claw, in test-tube studies only, looks like it might slow that enzyme down a bit. In theory, that could let a little more of the active steroid hang around than usual.
Honestly, I wouldn't lose sleep over this one. The evidence is lab-based, not from real people, and inhaled ciclesonide already puts very little drug into your bloodstream. Even if levels nudged up, you'd more likely notice mild things like a hoarse voice or mouth irritation than anything serious. Still, it's worth a quick mention to your pharmacist or doctor before you start devil's claw.

Alvesco (ciclesonide oral inhalation) is used for maintenance treatment of asthma. It is a preventive medicine for adults and teens 12 years of age and older.
It is not a rescue inhaler. It does not relieve sudden breathing trouble (acute bronchospasm) and is not indicated for children under 12.
