Devil's Claw and Digoxin

Here is the interaction we found between these two, with the evidence behind it.

Minor

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Digoxin

Digoxin falls under P-GLYCOPROTEIN SUBSTRATES — the group Devil's Claw interacts with.

Theoretically, devil's claw might increase levels of P-glycoprotein substrates.
In vitro research shows that devil's claw inhibits P-glycoprotein, which transports many drugs out of cells. This might increase intracellular levels of P-glycoprotein substrates, although it is unclear if this effect would be clinically important.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with these two together. Your body uses a kind of cellular pump called P-glycoprotein to push digoxin back out and keep its blood levels in check. In lab studies, devil's claw appears to slow that pump down. In theory, that could let a little more digoxin hang around in your system, nudging levels upward. Since digoxin has a narrow safety window, even small bumps can matter, and too much can cause nausea, vision changes (like a yellow-green tint), dizziness, or an off heartbeat.

That said, this concern comes only from test-tube research, not from people, so we honestly don't know if it matters in real life. Please don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding devil's claw.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Digoxin page

“Digoxin is a heart medicine with a narrow therapeutic index, meaning the gap between a helpful blood level and a harmful one is small, and few drugs give me more pause when supplements enter the picture. The most serious entries here are herbs that themselves contain digoxin-like cardiac glycosides, foxglove and lily-of-the-valley amon…”

About Digoxin

What Is Digoxin Used For?

Brand names: Lanoxicaps, Lanoxin, Digitek

Digoxin treats mild to moderate heart failure in adults. It improves heart failure symptoms and exercise capacity, and it reduces heart failure-related hospitalizations and emergency care. It does not change survival.

It also helps control the resting heart rate in adults with chronic atrial fibrillation (an irregular heartbeat). In children with heart failure, the tablet, oral solution and most injection products are used to increase the heart’s pumping strength (myocardial contractility). Some older injection labels list only the adult uses. Digoxin Oral Solution should not be used for multifocal atrial tachycardia.
