Devil's Claw and Vismodegib

Here is the interaction we found between these two, with the evidence behind it.

Minor

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Vismodegib

Vismodegib falls under P-GLYCOPROTEIN SUBSTRATES — the group Devil's Claw interacts with.

Theoretically, devil's claw might increase levels of P-glycoprotein substrates.
In vitro research shows that devil's claw inhibits P-glycoprotein, which transports many drugs out of cells. This might increase intracellular levels of P-glycoprotein substrates, although it is unclear if this effect would be clinically important.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Erivedge (vismodegib). Vismodegib is moved around your body partly by a cellular pump called P-glycoprotein, which helps shuttle the drug out of cells. Lab studies hint that devil's claw may slow that pump down. If that happened in real life, a bit more vismodegib could linger inside cells, which in theory might nudge up its common side effects like muscle cramps and spasms, hair thinning, loss of taste, nausea, or fatigue.

I want to be honest about the evidence here: this is only test-tube data, the interaction is rated minor, and we genuinely don't know if it matters in people. Since vismodegib is a serious cancer medication, please run any supplement, including devil's claw, past your oncologist or pharmacist before starting it.

This page covers Devil's Claw only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Vismodegib page lists 57 supplements it has a documented interaction with, worst first, each one explained.

The most serious there right now: Activated Charcoal (major), St. John's Wort (major), Bitter Melon (moderate), Black Pepper (moderate) and Bulbine Natalensis (moderate).

By severity: 2 major · 38 moderate · 17 minor.

From our pharmacist’s overview on the Vismodegib page

“St. John's wort can quietly undercut vismodegib (Erivedge), the pill for advanced basal cell carcinoma, by speeding up the gut pump that clears the drug from the body, and a cancer treatment that silently loses potency is the worst kind of interaction.”
