Erythromycin Ethylsuccinate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Devil's Claw interacts with.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two together. Your body uses an enzyme called CYP3A4 to help break down erythromycin. In lab studies (test tube only, not in actual people), devil's claw appeared to slow that enzyme down a bit. If that were to happen in your body, erythromycin might clear a little more slowly and build up, which could mean more of its usual side effects like nausea, stomach cramping, or diarrhea.
I want to be honest though: this is a theoretical concern based only on lab work, and it hasn't been seen in real patients. So I wouldn't lose sleep over it. Still, since erythromycin can affect heart rhythm in rare cases, it's worth a quick chat with your pharmacist or doctor before combining them.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that devil's claw inhibits P-glycoprotein, which transports many drugs out of cells. This might increase intracellular levels of P-glycoprotein substrates, although it is unclear if this effect would be clinically important.

Here's the gist with these two: your body uses a little 'pump' called P-glycoprotein to push certain drugs, including erythromycin, back out of cells. In lab dish studies, devil's claw appeared to slow that pump down. In theory, that could let a bit more erythromycin hang around inside your cells, which might nudge up its usual side effects like stomach upset, nausea, cramping, or diarrhea.
I want to be honest, though: this is only test-tube evidence, and we don't know if it actually matters in real people. So I wouldn't lose sleep over it. Still, since erythromycin already tends to irritate the gut, it's worth a quick mention to your pharmacist or doctor before pairing the two.
