Here's the situation with Opsynvi, which combines macitentan and tadalafil. Devil's claw has been shown in test-tube studies to possibly slow down a liver enzyme called CYP2C9, one of the enzymes your body uses to clear these medicines. If that slowdown happened in real life, these drugs could build up a bit more than expected, which might mean stronger effects like lower blood pressure, dizziness, lightheadedness, headache, flushing, or swelling.

The important part: this is only a theoretical concern seen in lab research, and it has never actually been reported in people. So I wouldn't lose sleep over it. Still, since Opsynvi already affects blood pressure, it's worth mentioning devil's claw to your pharmacist or doctor before starting it, just so someone can keep an eye on things.

Opsynvi combines two medicines, macitentan and tadalafil, and both are broken down in your body partly by an enzyme called CYP3A4. Here's the theory behind this pairing: devil's claw might slow that enzyme down a bit. If it did, these medicines could clear more slowly and build up, which could nudge up their effects and side effects. For tadalafil that can mean headache, flushing, stuffy nose, or lightheadedness, and since both drugs can lower blood pressure, you might feel dizzy when standing.

The important part is that this is only seen in lab (test tube) research, not in real people, so I wouldn't lose sleep over it. Still, it's worth a quick word with your pharmacist or doctor before adding devil's claw.
