Fam-Trastuzumab, Deruxtecan-Nxki falls under P-GLYCOPROTEIN SUBSTRATES — the group Devil's Claw interacts with.

Theoretically, devil's claw might increase levels of P-glycoprotein substrates.
In vitro research shows that devil's claw inhibits P-glycoprotein, which transports many drugs out of cells. This might increase intracellular levels of P-glycoprotein substrates, although it is unclear if this effect would be clinically important.

Here's the gist with Enhertu (fam-trastuzumab deruxtecan), a powerful infused cancer therapy. In a test-tube setting, devil's claw seems to block a cellular pump called P-glycoprotein, which helps move certain drugs out of your cells. Enhertu is considered a substrate of that pump, so in theory, slowing the pump could let a little more of the drug linger inside cells, possibly nudging up side effects like nausea, fatigue, low blood counts, or, rarely, lung inflammation.

I want to be honest though: this is only lab-based evidence, the interaction is rated minor, and we don't know if it matters in real people. Still, because Enhertu is serious chemotherapy, please check with your oncologist or pharmacist before adding devil's claw.

What Is Fam-trastuzumab deruxtecan-nxki Injection Used For?

Brand name: Enhertu

Enhertu (fam-trastuzumab deruxtecan-nxki) is used to treat adults with certain HER2-positive, HER2-low, or HER2-ultralow cancers. It covers a wide range of situations — from early-stage breast cancer before or after surgery, to metastatic (spread) breast cancer, to lung, stomach, and other solid tumors.

Specifically, Enhertu is approved for: HER2-positive early breast cancer (as neoadjuvant treatment before surgery, or as adjuvant treatment after surgery when cancer remains); unresectable or metastatic HER2-positive breast cancer (alone or with pertuzumab); metastatic HER2-low or HER2-ultralow hormone receptor-positive breast cancer; HER2-mutant non-small cell lung cancer; HER2-positive locally advanced or metastatic gastric or gastroesophageal junction cancer; and HER2-positive (IHC 3+) unresectable or metastatic solid tumors with no satisfactory alternatives. Some of these uses are approved under accelerated approval and may require further confirmation of benefit.
