Fluticasone Proprionate, Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Devil's Claw interacts with.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with your Airduo (fluticasone plus salmeterol) inhaler. Both medicines are broken down in the body by an enzyme called CYP3A4. In lab dish studies, devil's claw appears to slow that enzyme down a bit. If that happened in real life, it could let a little more of these drugs build up rather than being cleared normally.
For fluticasone, higher levels can mean more steroid effects (things like a hoarse voice, oral thrush, or with a lot of buildup, body-wide steroid effects). For salmeterol, more drug could mean a faster heartbeat, jitteriness, or tremor. That said, this is only a theoretical, test-tube finding and hasn't been shown in people, so for most folks it's likely minor. Still, it's worth a quick mention to your pharmacist or doctor before adding devil's claw.
