Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Terminalia interacts with.
In vitro research shows that Terminalia arjuna extract inhibits CYP2D6 enzymes and reduces CYP2D6 substrate metabolism.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo: the codeine part of this product is a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Lab studies suggest Terminalia arjuna may slow that enzyme down. If less codeine gets converted to morphine, you could get less pain relief than expected from the codeine, even though it might leave a bit more codeine itself floating around.
The aspirin and caffeine pieces aren't really affected here. Keep in mind this is based only on test-tube research, not people, so it may not amount to much in real life. Still, if your pain isn't well controlled, that's worth mentioning to your pharmacist or doctor before changing anything.

This combination pill mixes aspirin, caffeine, and codeine. The piece that matters here is the codeine. Your liver uses an enzyme called CYP3A4 to help process part of it, and lab studies suggest Terminalia arjuna may slow that enzyme down a bit. In theory, that could let a little more of the drug linger, nudging up the usual codeine side effects like drowsiness, dizziness, constipation, or slowed breathing.
Keep in mind this is only test-tube evidence, not human data, so the real-world effect is uncertain and probably small for most people. Still, since codeine causes sedation, it is worth a quick chat with your pharmacist or doctor before pairing the two, especially if you take other drugs that make you sleepy.

ANTICOAGULANT/ANTIPLATELET DRUGS

In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.

Here's the thing to keep an eye on with this combo. Your medication contains aspirin, which already thins the blood a bit by keeping platelets (the cells that form clots) from sticking together. In lab studies, Terminalia arjuna seems to do something similar, slowing platelet activity. Stack the two and you could, in theory, nudge up your bleeding risk a little.
In real life that might show up as easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop. I'll be honest with you though, this concern comes from test-tube research, not solid human studies, so it's more of a caution than a proven problem. The codeine and caffeine parts aren't really involved here. If you take this regularly, just run the Terminalia past your pharmacist or doctor first.
