This combination pill has three parts, and the one that matters here is the aspirin. Aspirin already thins your blood by making platelets less sticky, which is exactly what helps prevent clots but also makes bleeding easier. Lab studies suggest onion compounds can do something similar and gently nudge platelets in the same direction. So in theory, pairing them could add up to a slightly higher chance of bruising, nosebleeds, or stomach bleeding.

Keep in mind this is theoretical and based on test-tube work, not real patient outcomes. Onion in your food is not a concern. Concentrated onion supplements are the real question. If you take aspirin daily or have any bleeding history, just check with your pharmacist or doctor before using them.

Here's a simple way to think about this one. Your medicine combines aspirin, caffeine, and codeine, but the interaction we have on file is specifically about the aspirin piece. This isn't about how onion changes the way your body breaks the drug down. Instead, the concern is allergy. In one single case report, a person who already had a mild onion allergy noticed it got worse (swelling and hives) after taking aspirin. Aspirin can sometimes amplify allergic-type reactions in sensitive people.

For most folks who eat onion in food and have no onion allergy, this is very unlikely to be a problem. The evidence here is just one anecdote, so it's weak. But if you know you react to onion, mention it to your pharmacist or doctor before using concentrated onion supplements alongside this medicine.
