Onion and Clopidogrel

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Clopidogrel

Clopidogrel falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Onion interacts with.

Theoretically, concomitant use of anticoagulant or antiplatelet drugs with onion might increase the risk of bleeding.
In vitro research shows that onion inhibits platelet aggregation.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: Clopidogrel works by keeping your platelets from sticking together, which lowers your risk of clots after a stent, heart attack, or stroke. Onion, in lab studies, seems to gently nudge platelets in the same direction, making them a little less sticky. Stack those two effects and the theory is that bleeding or bruising could be slightly more likely.

I want to be honest about how soft this concern is. It's based on test-tube research, not real patient outcomes, so it's truly theoretical. Onions on your burger or in your soup are not going to be a problem. Concentrated onion supplements are where I'd pause. If you notice easy bruising, nosebleeds, or bleeding that won't stop, or you're considering an onion supplement, check with your pharmacist or doctor first.

Onion is a common food with a long history in traditional medicine, and it contains antioxidants like quercetin and sulfur compounds. Eating onion as part of a balanced diet is safe for most people, but high-dose supplements have only limited human evidence for any specific health condition. Talk with your pharmacist or doctor before using concentrated onion products for a medical reason.
