Why Are There 2 Interactions?

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Aspirin, Caffeine, Hydrocodone

Aspirin, Caffeine, Hydrocodone falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Onion interacts with.

Theoretically, concomitant use of anticoagulant or antiplatelet drugs with onion might increase the risk of bleeding.
In vitro research shows that onion inhibits platelet aggregation.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with Damason-P (a combination of aspirin, caffeine, and hydrocodone): the aspirin part already thins your blood a bit by making your platelets less sticky. In lab studies, onion compounds like quercetin and its sulfur compounds seem to do something similar, slowing platelet clumping. Put them together and, at least in theory, you could nudge up the chance of bleeding or bruising, such as nosebleeds, bleeding gums, or easy bruises.

I want to be honest though, this is a theoretical concern based on lab research, not strong human studies. Normal onion in your cooking is very unlikely to cause trouble. Concentrated onion supplements are the bigger question mark. If you take those regularly, just run it by your pharmacist or doctor first.

ASPIRIN

Concomitant use of aspirin with onion may worsen onion allergy.
In one case report, a patient with a mild onion allergy reported worsening allergy, including swelling and severe urticaria, after taking aspirin.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the deal with this combo, which pairs the aspirin in Damason-P with onion. This isn't about how your body breaks down the medicine. It's about allergy. We have a single case report of someone with a mild onion allergy whose symptoms got noticeably worse, including swelling and severe hives (urticaria), after taking aspirin. The thinking is that aspirin may amp up an existing onion sensitivity.

So if you've never reacted to onions, eating them with this medication isn't a real concern. But if you already know onions bother you, be alert for swelling, itching, or hives. This is anecdotal, weak evidence, so don't panic, but do mention any onion allergy to your pharmacist or doctor.

Onion is a common food with a long history in traditional medicine, and it contains antioxidants like quercetin and sulfur compounds. Eating onion as part of a balanced diet is safe for most people, but high-dose supplements have only limited human evidence for any specific health condition. Talk with your pharmacist or doctor before using concentrated onion products for a medical reason.
