Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Onion interacts with.
In vitro research shows that onion inhibits platelet aggregation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the connection with Ventavis (iloprost). Besides relaxing the blood vessels in your lungs, iloprost also has a mild blood-thinning side, since it can keep platelets from clumping together. Onion appears to do something similar in lab dishes, gently slowing platelet activity. Stacked together, the worry is a slightly higher chance of bleeding or bruising, things like nosebleeds, bleeding gums, or marks that show up easily on your skin.
I want to keep this in perspective, though. This is a theoretical concern based on lab work, not solid human studies, and onion in normal food amounts is very unlikely to cause trouble. Concentrated onion supplements are the bigger question mark. If you'd like to take those, run it by your pharmacist or doctor first.

Specifically, it is used to reduce the risk of losing fingers or toes (digit amputation) after serious frostbite injury. The clinical evidence supporting this use comes from young, healthy adults who suffered frostbite at high altitudes.
