Moringa and Atogepant

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Moringa interacts with whole categories of medication. Atogepant falls into 2 of those categories, so it can interact with Moringa in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Atogepant

Atogepant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with moringa and Qulipta (atogepant). Your body relies on a liver enzyme called CYP3A4 to clear atogepant out of your system. The lab research on moringa is genuinely mixed: some test-tube studies suggest it slows CYP3A4 down, while others suggest it speeds it up. So we honestly can't say for sure which direction it would push your atogepant levels.

If moringa slowed that enzyme, atogepant could build up and you might notice more side effects like nausea, constipation, or sleepiness. If it sped the enzyme up, the drug could clear too fast and work less well at preventing your migraines. Keep in mind this is theoretical and comes from lab dishes, not people, and one human study showed no real effect. Food amounts of moringa are unlikely to matter, but if you're using concentrated supplements, mention it to your pharmacist or doctor.

P-GLYCOPROTEIN SUBSTRATES

2 of 2

Atogepant also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Qulipta (atogepant). Your body uses a little 'pump' called P-glycoprotein to move this drug around and help clear it. In lab studies, moringa leaf extract slowed that pump down. If that happened in a person, less atogepant would get pumped out, so more could build up in your system. That might mean stronger side effects like nausea, constipation, tiredness, or loss of appetite.

The honest part: this is theoretical. It's only been seen in test-tube research, never reported in real people, so I don't want you to panic. Everyday cooking amounts of moringa are unlikely to matter much. Concentrated supplement capsules are the bigger question mark. Just let your pharmacist or doctor know before you pair the two, and don't change your migraine medicine on your own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Atogepant page

“Grapefruit is the single interaction I would want every Qulipta user to know first. Atogepant, a preventive migraine medicine, is cleared by a liver enzyme called CYP3A4, and grapefruit juice blocks that enzyme, so the drug can build up and side effects become more likely.”

About Moringa

What is Moringa?

  • Nutritional supplement
  • Blood sugar support
  • Cholesterol support
  • Inflammation
  • Skin conditions

Moringa is a nutrient-rich plant whose leaves are widely used as a food and supplement, especially in parts of the world where malnutrition is common. Early research hints at possible benefits for blood sugar, cholesterol, and inflammation, but the human evidence is limited and not strong enough to recommend it as a treatment. Talk with your pharmacist or doctor before using moringa, especially if you are pregnant, breastfeeding, or taking medications.
