Why this applies to Apalutamide

Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Here's the honest picture with moringa and Erleada (apalutamide). Your body processes apalutamide partly through a liver enzyme called CYP3A4. The concern on file is that moringa might nudge this enzyme either up or down. If it slowed the enzyme, apalutamide could build up a bit, which might mean more fatigue, joint pain, rash, or other side effects. If it sped the enzyme up, the drug might clear faster and work less well against the cancer.

The catch is that this comes only from lab and test-tube studies, and even those disagree on which direction it goes. One human study with a different drug showed no real change. So this is mostly theoretical, not a proven problem. Food amounts in cooking are unlikely to matter, but please check with your oncology team before using concentrated moringa supplements alongside such an important medication.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
